Valley Community Services Board is an equal opportunity employer and a drug free workplace.   Open Date 12/8/2023   Close Date 1/4/2024   Position Case Manager, Permanent Supportive Housing (ACT)   Full-Time/Part-Time Full-Time   Exempt/Non-Exempt Exempt   Shift Days   Req Number CAS-23-00032   Number of Openings 1   Number Filled 0   Description As a member of the Assertive Community Treatment (ACT) team, the Case Manager for Permanent Supportive Housing (PSH) will serve as primary provider of assertive community treatment for individuals with serious mental illness (SMI) or dually diagnosed consumers participating in Valley Community Services Board's PSH program. The incumbent collaborates with ACT teammates to enhance psychiatric stability, quality of life, access to available resources, and appropriate use of medical services and inpatient care. Benefits: Sign-on bonus of $1,500.00 included in full with the first paycheck. Hybrid work environment. Health Insurance as low as $20.00/month for individuals. Family and dependent plans are also available. Nine paid holidays annually. Accrue up to 20 days of paid time off (PTO) per year from year one. PTO is immediately available upon accrual. Participation in the Virginia Retirement System. Summary of Essential Functions Understand and embrace the Housing First principles and engage in a housing-focused case management approach to service delivery. Providing care under the ACT model through supportive counseling, relationship skills development, psychiatric rehabilitation counseling, symptom management, housing support, and vocational readiness and placement. Proactive and ongoing client assessment, outreach, and advocacy. Developing and administering person-centered treatment plans. Provide crisis intervention support and coordination for individuals experiencing acute distress. Minimum Qualifications In order to be considered for the position, all applicants must meet all of the following criteria. Bachelor's Degree in Human Services or related field from an accredited program, One year of direct clinical experience providing services to individuals with mental illness, and Valid Virginia driver's license with an acceptable driving record.   Position Requirements     Salary Information   EOE Statement Valley Community Services Board is an equal employment opportunity employer.
