Valley Community Services Board serves as the single point of entry into publicly funded mental health, developmental, and substance use disorder services for these localities.

Description

As a member of the Assertive Community Treatment (ACT) team, the Case Manager for Permanent Supportive Housing (PSH) will serve as primary provider of assertive community treatment for individuals with serious mental illness (SMI) or dually diagnosed consumers participating in Valley Community Services Board's PSH program. The incumbent collaborates with ACT teammates to enhance psychiatric stability, quality of life, access to available resources, and appropriate use of medical services and inpatient care.

Summary of Essential Functions

  • Understand and embrace the Housing First principles and engage in a housing-focused case management approach to service delivery.
  • Providing care under the ACT model through supportive counseling, relationship skills development, psychiatric rehabilitation counseling, symptom management, housing support, and vocational readiness and placement.
  • Proactive and ongoing client assessment, outreach, and advocacy.
  • Developing and administering person-centered treatment plans.
  • Provide crisis intervention support and coordination for individuals experiencing acute distress.

Minimum Qualifications

In order to be considered for the position, all applicants must meet all of the following criteria.

  • Bachelor's Degree in Human Services or related field from an accredited program,
  • One year of direct clinical experience providing services to individuals with mental illness, and
  • Valid Virginia driver's license with an acceptable driving record.
