This community-based Case Management position primarily aims to reach out to the community to help facilitate those in need of treatment. The Care Manager will be responsible for working with the local community to engage individuals with opiate use disorders to facilitate ongoing treatment and provision of other needed services to engage them and help them to remain engaged.   Position Requirements BSW or MSW. Must have knowledge of Addiction and MOUD (Medication for Opioid Use Disorder). Ability to navigate multiple systems and be able to multi-task. Experience with case management. Knowledge of community resources. Must have reliable transportation and a valid driver’s license.   This position is currently not accepting applications.
