Duties involve providing supportive assistance as clients navigate medication-assisted treatment, as well as completing monthly treatment planning reviews and billing. Must exude enthusiasm, sound judgment, and effective communication, with time management skills to successfully deliver services. Minimum requirements- CSAC or CSAC-A. Fulltime position with benefits.   The individual in this position provides direct care coordination services to individuals enrolled in the Office Based Addiction Treatment services, and their families. Primary services include providing a range of ongoing community substance use care coordination services to individuals with an Opioid Use Disorder within the Staunton/Waynesboro, Augusta, and Highland County catchment areas; and providing monthly chart auditing to support Care Coordination billing. Services include but are not limited to: assessment and treatment planning, linking and coordinating internal and external services and supports, monitoring, crisis intervention, screening for admission to residential programs and discharge planning, and ensuring Office Based Addiction Treatment clients are informed of the OBAT process. He/She is responsible for carrying out client treatment plans developed from initial and ongoing assessment and clinical staffings and submitting for monthly billing. He/she is responsible for documenting to Medicaid and licensure standards and adhering to professional standards of ethical treatment. He/She adheres to confidentiality guidelines, respects integrity, and protects the rights and welfare of individuals. He/She is expected to facilitate positive consumer and peer relationships based on courtesy, compassion, and respect. The incumbent is responsible to and supervised by the Office Based Addiction Treatment Supervisor.   The OBOT Care Coordinator must meet DMAS "Credentialed Addiction Treatment Professional" criteria with either an active LMHP, LMHP-E, and/or CSAC certification.   Position Requirements   Salary Information   EOE Statement Valley Community Services Board is an equal employment opportunity employer.
