General Purpose: 

Implements and maintains accurate and timely client medication management, assessments, and screenings (i.e., initial suicide and nutritional screenings), facilitates medical discharge, promotes routine vaccinations, and promotes/offers relevant medical assistance and trainings to staff. This individual is expected to maintain a healthy staff-client relationship with all.

 

What I’m Responsible For (Essential Functions):

The following statements regarding this Non-Exempt level position consist of duties, responsibilities, and frequency describing the general nature and level of work performed by the employee filling this position. These statements are not an exhaustive list of all duties and responsibilities required by this position.   • Professionally and accurately complete client medication management, general first aid, and nursing support.
  •  Assist with medication self-administration program and provides on-going quality assurance monitoring and training as needed.
  • Assist in administering and monitoring the Tuberculosis and Infection Control Program.
  • Demonstrates a good understanding of individual client needs, abilities, and feelings including both developmental and cultural needs. Aware of emotions and behaviors typical of children who have been abused, neglected, and traumatized.
  • Participate in the on-going assessment of the health and safety policies of the agency.
  • Accurately create and maintain a current and accurate medication list for all clients.
  • Effectively navigate client records to access information necessary to schedule medical appointments and/or complete provider paperwork.
  • Accurate and timely completion of records, filing, forms, and other electronic systems based on input from various sources including providers, medical, and/or direct care staff.
  • Understand and comply with HIPAA regulations and maintain the confidentiality of CHC clients and employees in all situations including outside of normal business hours.
  • Complete reports on a variety of healthcare processes as needed.
  • Recognize and comply with unique security and safety requirements of the department to include notifying on-call staff when necessary.
  • Perform physical holds/restraints as needed.
  • Knows “Six Rights” of the clients in the self-administration of medication process.
  • Maintain appropriate level of professionalism regarding boundaries and relationships with clients.
  • Professionally communicate with staff, clients, outside vendors both verbally and in writing.
  • Attend meetings as needed and as directed by the supervisor.
  • Other duties, responsibilities, and/or activities may change or be assigned at any time with or without notice.

  • Administration/Documentation - Complete written correspondence, documentation, and data entry in an accurate and timely manner. Efficiently prepare reports, forms, and other paperwork as required. Demonstrate ability in planning and implementing long- and short-range department initiatives.
  • Client Health and Safety - Reduce, enforce, and comply with environmental, personal, and other factors that pose a risk to clients.
  • Collaboration - Engage in therapeutic and professional relationships in completing projects, tasks, and development of processes.
  • Communication/Problem Solving - Demonstrated organizational skills and other complex activities in support of agency, department, and position objectives. Provide complete, thorough, and detailed guidance with staff, clients, and agency leadership.
  • Attending, lead, and contribute to meetings as needed. Coordinate and communicate with internal and external departments/individuals on client issues. Listens actively and understands others, finds solutions when problems arise and offers alternative solutions, when necessary, Asks appropriate questions when not sure of job requirements or duties.
  • Initiative - Complete assigned tasks and duties successfully with little to no supervision in a timely manner.
  • Leadership - Practice and model behavior consistent with agency’s code of professional conduct and other policies while also demonstrating ethical practice.
  • Professional/Occupational - Demonstrate appropriate behaviors and expectations. Lead by example. Ability to exercise good judgment, to demonstrate an understanding of ethics related to development activities, and to use discretion in interactions with others. Familiar with, interprets, and follows agency policy and state laws regarding employment, workplace, and clients.
  • Promote a healthy, therapeutic milieu - Foster and encourages a structured environment that creates a safe, secure place for clients.
  • Teamwork and Positive Interaction – Reflect positive people skills and the ability to interact effectively with staff, clients, leadership, stakeholders, and community members in a wide range of roles. Able to work cohesively with all departments fostering effective working relationships within a team environment.
  • Trauma Informed Care - Demonstrate awareness and understanding of trauma informed care framework and integrates strategies that are consistent with framework.

Physical Demands and Work Environment:

Light physical demands primarily performed indoors with limited exposure to changes in
weather and fluctuation in temperature. Position requires minimal physical activity including moderate periods of walking, standing, bending, and light to moderate lifting. Position requires periods of stationary activity/sitting. May travel or walk where exposure to the elements occurs. Physical contact while using physical de-escalation holds with clients is possible requiring the use of arms, hands, legs, and fingers. The noise level in the work environment is usually moderate to loud.

 

 
Position Requirements

Minimum Requirements:

  • Must be a graduate of an accredited and board approved nursing program.
  • Must have a current, valid RN license from the State of Wyoming
  • Strong written communication skills with a proven background in interpersonal, organizational, time-management, oral communication, and teamwork.
  • Experience and application in a variety of MS Office Suite and agency specific technology or equipment.
  • Minimum 21 years of age at time of hire.
  • Driving record of no more than two moving violations or at fault accidents in any combination in the past three years, and no major moving violations within the past three years.
  • Clean background and child abuse registry checks.

Preferred Requirements:
Education and Experience and Background:
• Associate degree in Nursing preferred AND minimum of one year experience working with adolescent populations and families with mental/behavioral health needs.
• Clinical documentation, medical billing, or technical writing skills.
• Strong skill set pertaining to rapport building, active listening, and therapeutic interactions
