The ACT Permanent Supportive Housing Case Manager will be working with a population of adults who may be dually diagnosed with chronic mental illness and substance use issue. The incumbent provides intensive case management, rehabilitation, and supportive services to individuals experiencing serious mental illness. This incumbent will also assist the formerly homeless individuals maintain their housing within the permanent housing program. Services are provided in accordance with the ACT model of treatment and Housing First Principles. Services are provided in multiple settings, including in the home and in the community.   The ACT PSH position is full-time and requires working a schedule rotation of evenings, weekends, and holidays. There are limited on-call responsibilities. Virginia driver’s license in good standing is required. Bachelor’s Degree in a human services field with at least one year of clinical experience working with adults experiencing chronic mental illness and possess an understanding of Housing First principles.
