Referral Specialist/Medical Records Clerk

Community First Health Centers currently has a Referral Specialist/Medical Records Clerk opportunity available at our Port Huron medical facility. The ideal candidate for this role will be enthusiastic, motivated and a team player. Under the supervision of the Practice Manager, the ideal candidate will assist with patient referrals, medical records requests and providing excellent customer service. This position will be located out of our Port Huron, MI medical facility located at 1011 Military Street, Port Huron, MI .

  • Promotes and maintains awareness of agency programs, services and activities
  • Maintains communication between medical staff, patient(s) and insurer(s)
  • Participates in Quality Improvement activities as needed/directed
  • Assists with patient referral(s) based on requirements of insurance and HMO companies
  • Assist patient(s) with setting up referral(s) and diagnostic testing; tracking of all referrals and diagnostic testing
  • Complete all prior authorization(s) needed for referral(s) and diagnostic testing; perform prior authorization(s) for patient medication as needed/directed
  • Attend meetings, seminars, workshops and conferences, as needed and approved for professional development and on behalf of Community First Health Centers
  • Assist in the development and generation of referral related or other requested reports
  • Scan patient health information into the electronic medical record.
  • Copy and send all record requests from patients, physician's office, lawyer's offices', suubpoena's from insurance companies, etc.
  • Other duties as assigned


This is a full time; 40 hours per week position.

Hours will be gained predominately Monday thru Friday; 830am to 5pm but some flexibility will be needed based on business demands.

Position Requirements
  • High School Diploma or General Education Degree (GED) required
  • Two years of health care experience
  • Familiarity with medical terminology preferred
  • Familiarity with medical records preferred
