Job Summary

Provides community and public health nursing services in the clinic, community and patient homes in accordance with established policies and procedures with an emphasis on education and prevention. Duties include adult and child care clinics and communicable disease control (i.e., STD, HIV/AIDS, etc.) Patient referrals/follow-up, home visits, individual and group conferences, health promotion and illness prevention activities, patient health counseling, assessments to determine and appraise public health status and needs. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements.

Essential Job Functions:

  1. Collaborate in development and delivery of programs and activities for individuals, families and population groups that promote health and prevent disease, in settings including, but not limited to homes, community organizations, businesses, schools, and community events.
  2. Participate in the assessment and monitoring of population health using systematic, evidence-based processes to solve community health problems.
  3. Ensure the delivery of communicable disease services including screening, case investigations, treatment, and contact tracing in alignment with Oregon Health Authority recommended investigative and treatment guidelines.
  4. Provide education and counseling while using appropriate and professional methods of communication to individuals, families and community groups that are adapted to their unique needs, lifestyles, physical abilities, sexual orientations, cultural and socio-economic situations, and racial and ethnic backgrounds Work closely with the primary care provider teams to coordinate care, ensure consistent communication concerning patient health status, care plans, and outcomes of interventions.
  5. Maintain confidentiality in accordance with Yellowhawk Tribal Health Center Personnel Manual, policies, and HIPAA.
  6. Represents Yellowhawk with a respected and viable presence at community events, within the clinic, and while in attendance at conferences, courses, and events outside of the CTUIR boundaries.
  7. Actively participate in the monitoring of the high-risk elders case list via monthly meetings and ensure appropriate individuals are represented through advocacy and communication with all parties/entities vital to the safety and care of the elder population.
  8. Provide home visits per policy as ordered by primary care providers and in accordance to documented care plans.
  9. Participate in the planning of health education campaigns and disease prevention activities that involve improving access among community members to clinical care services.
  10. Performs public health emergency response duties as assigned and consistent with job classification and training provided.
  11. When assigned case management assess, implement and evaluate patient status using the standards of nursing care and appropriate delegation of tasks to Community Health Representatives.
  12. Assists Public Health and Medical teams with immunization management and coordination, including providing immunizations to community members of all ages upon request, by appointment, or during immunization clinics.
  13. Deliver targeted, culturally-appropriate information to help individuals, groups, and entities understand local environmental public health policies, regulations and code.
  14. Assist in the design, production, and dissemination of collateral marketing materials (letters, brochures, and flyers) in accordance with the Yellowhawk Public Relations Policy.)
  15. Work with the Public Health Operations Supervisor to educate local and state policy makers and community stakeholders on public health issues which are relevant to Confederated Tribes of Umatilla Indian Reservation.
  16. Adheres to ethical principles and Yellowhawk Tribal Health Center policy in the collection, maintenance, use, and dissemination of data and information.
  17. Use quality improvement (QI) and performance management (PM) processes and/or techniques to improve the effectiveness of the respective public health program. This includes, but is not limited to: creating, implementing, and evaluating performance standards and identifying, implementing, and assessing program quality improvement processes.
  18. Provide monthly reports to appropriate department, management, and governmental entities as needed and requested.
  19. Other duties as assigned.
 
Position Requirements

Minimum Qualifications:

  1. Registered Nurse with an Associate Degree in Nursing with one year successful experience and/or training in Public (or Community) Health Nursing.
  2. Must have active, current license as a registered nurse in the State of Oregon.
  3. Demonstrated ability to communicate effectively, both in writing and orally, in any situation, with peers, staff, management, local, state and federal agencies and the general public, on various topics including, but not limited to policies, procedures and general operations.
  4. Demonstrated ability to work independently with minimal supervision, plan and organize daily work schedule, identify problems and recommend solutions.
  5. Demonstrated knowledge of community health representative/worker scope of practice.
  6. Demonstrated ability to handle conflict and crisis in a professional manner.
  7. Demonstrated experience with computer software, e.g., Microsoft Office, Electronic Health Records, RPMS.
  8. Demonstrated excellent interpersonal skills.
  9. Must be willing to attend Yellowhawk sponsored and led community events and clinics in the evenings and occasionally on weekends outside of normally assigned work hours.
  10. Must have and maintain CPR/BLS certification.
  11. Must possess a valid state driver’s license and meet and maintain insurance requirements to operate General Services Administration vehicles.
  12. Must submit to and pass a reference and criminal background check.
  13. Must submit to and pass a pre-employment drug and alcohol screening.
  14. Must submit proof of COVID-19 vaccination.

Preferred Qualifications:

  1. Registered Nurse with BSN Degree and training in Public (or Community) Health Nursing preferred.
  2. Experience working out in the community as well as in the patient’s home.
  3. Experience and/or knowledge of history, culture and health needs of Native American communities preferred.
  4. Experience in a Tribal healthcare system
