The social worker is responsible for the implementation of standards of care for medical social work services for adherence to all conditions in the Service or Employment agreement. Will assess the psychosocial status of patients, carry out social evaluations and plan intervention based on evaluation findings, counsel patients and identify coping difficulties. Will maintain clinical records on all patients, provide information and referral services, provide information to patients and families/caregivers, serve as a liaison between patients and families, maintain and develop contracts with agencies as resources, participate in the development of individualized care plans, assist physicians and other team members in understanding significant factors related to health problems, participate in quality assessments and other duties as designated.   Position Requirements Requires bachelor's degree in social work, minimum one year experience in health care or hospice (hospice experience preferred), LSW.   This position is currently not accepting applications.
