Southeast Healthcare is looking for a dedicated Assertive Community Treatment (ACT) nurse to provide community-based services to adults with severe mental disabilities. The nurse provides medication/medical/mental health education and direction to non-medical team members and clients, as well as nursing services per family practice physician orders and communicates with physicians and health services outside Southeast to assure quality and coordination of care. Position works 3 days/week with the ACT team and 2 days/week in our primary care clinic.   Position Requirements Ohio LPN/RN license required. As a healthcare provider, Southeast Healthcare is a non-smoking employer.  Staff members are required to be vaccinated for the flu and COVID.  Religious and medical exemptions may be requested during the hiring process.
