Description

Social Work p.r.n. has an immediate need for a LMSW or LSCSW to serve a 60-90-day, full-time, temporary contract for a growing health care program that helps people at risk for needing nursing home stay in their own homes and communities. The purpose of the position is to provide services to meet the social and/or emotional needs that affect the participants’ ability to achieve their goals. Program participants live in the Wyandotte County area; however, candidates can complete most job responsibilities at home other than assessments that are in-person.

Job responsibilities include:

  1. Assess the ongoing psychosocial needs of participants/families based on emotional, environmental, financial, psychological, cognitive, social and health factors and access community resources when applicable.

  2. Collaborate with an interdisciplinary team in the development and ongoing evaluation of a psychosocial care plan.

  3. Involve the patient/family in determining care goals and objectives.

  4. Recognize signs and symptoms of psychiatric illness and provide therapeutic interventions to address the stages of grief and loss.

 
Position Requirements

Candidates must have a masters degree from an accredited social work program and licensure in Kansas (LMSW or LSCSW).
