ProMedica Hospice, formerly Heartland Hospice, has an exciting opportunity for a full-time week day RN Case Manager (very limited on call after hours required, every 5th weekend and 1 holiday/year) to join our established growing hospice agency caring for patients in the Fox Valley area. As a RN Case Manager, you will supervise and coordinate interdisciplinary care delivery to our home bound hospice patients and provide clinical visits throughout the weekdays. In addition, you will possess excellent written and verbal communication skills, good problem solving and decision-making skills and at least 6 months institutional nursing experience. Hospice experience is not needed as will train the right candidate. If you are compassionate and want to utilize your nursing skills in a way that will personally touch your patients, this is the position for you!
