The Case Manager will play a vital role in supporting individuals in need by providing comprehensive case management services. This position requires a strong focus on working directly with individuals from opiate-impacted communities, collaborating with community partners, and actively engaging in outreach efforts.   Position Requirements BSW - Demonstrated experience in case management, social services, or a similar role is preferred.
