Facilitate individual and group sessions per assigned clients and monitor the client's compliance per client level of care for a medication-assisted treatment program. Attend Clinical Case Consultation, clinical meeting, staff meeting, and clinical supervision as scheduled. Provide crisis intervention. Assist in client aftercare planning and follow-up. Facilitate the therapeutic process. Evaluate the client's progress and report to funders/referral sources as requested. Prepare case presentations for assigned clients at Clinical Case Consultation. Determine resources needed to meet the individualized needs identified in the assessment process and coordinate community support activities. Take all necessary actions to ensure the implementation of a comprehensive service plan (treatment plan) including serving as an advocate for the individual to enable them to receive needed services; identification of program needs, and service gaps, and recommending strategies.   Position Requirements BSW or MSW and one year of experience are required.
