Established in 1951, our organization supports over 8,600 individuals (whom we refer to as Participants) with a variety of disabilities. A leading provider of disability services, ESL has physical locations in 8 regions of Louisiana and our array of programs include; support coordination (case management) for children, adults and elderly participants, a wide range of behavioral health and housing services, early intervention services for infants and toddlers, and a school setting called The Chartwell Center in New Orleans.

Job Description:

Easterseals of Louisiana has a new partnership and we need a dynamic person to join our team and help us build this new program. We are seeking a Case Manager in our Housing division.   • Apply the Housing First approach to serving people with housing and other service needs.
  • Will engage high-risk individuals with a focus on successfully linking them to needed higher level of care.
  • Participates as an active member of Unity of GNO's Service Providers and Professionals Association (SPPA).
  • Submit routine reports and outcome data, per contractual requirements, will utilize HMIS system.
  • Attend and participate in weekly staff meetings.
  • Attend and participate in required training.
  • Work with surviors of doemestic violence.
 
Position Requirements

Qualifications (please take note, we have no flexibility on degrees accepted. These are conditions of our contractor):

  • a Bachelor’s Degree in a Human Services related field (i.e. Sociology, Psychology, Education, Counseling, Social Services, Family & Participant Science, Rehabilitation Services, or equivalent.)
  • Reliable transportation so that you may make visits to the field to conduct participant visits. You may anticipate about 50% or more of your time will be in the field visits.
  • Easterseals Louisiana is a Drug-Free Workplace, pre-employment and random drug tests will be administered.
  • Will be required to pass Easterseals Louisiana background check
  • Those who do not choose to be vaccinated and provide proof of vaccination, will be required to follow agency protocols.

Salary:$40,000-45,000
