Develop and maintain a Social Service Program for Cedarwood that meets all state and federal guidelines. Complete MDS, care plans, and social service documentation in a timely manner. Coordinate consultant vision, dental, podiatry, and audiology services. Review and update social service policies and procedures annually and as needed. Assist in issuing Medicare A and insurance cut letters. Serve as Grievance Official for the facility. Coordinate resident and family care conferences and TOC meetings in cooperation with RNAC. Participate as part of the Risk Management Committee. Perform a variety of services for meeting the psychosocial and personal needs of residents and families, such as assisting with initial adjustment, subsequent changes, trauma-informed care assessment, and crisis management. Financial counseling and help with the medical assistance application process. Oversee the discharge-planning process. Assist NHA and DON with the investigation of abuse allegations. Perform other duties as required.   Position Requirements BSW and experience in a health care setting preferred.   This position is currently not accepting applications.
