The social worker will guide program enrollees and their caretakers through the health care system by assisting with access issues, developing relationships with service providers and tracking interventions and outcomes.  BSW Annual Salary Range: $48,000-$52,000, MSW Annual Salary Range: $52,000-$56,000.   Position Requirements Requires bilingual (Spanish and English), BSW or MSW, and must have care management experience. The position requires home visits throughout NYC and work is done 3 days from the office and 2 days remotely.    This position is currently accepting applications.
