Job Title: Physical Therapist Floater
Locations Available: We are looking for a full-time Physical Therapist Floater who will travel to all of our Alden homes. On the clock when traveling SNF to SNF.
Status: Full time Monday ~ Friday 40 hours week

Job Responsibilities:
Adhere to the policies and procedures for the provision of therapy services in accordance with the goals of CPT and Facility
Maintain the standards for rehabilitation services
Perform administrative duties as necessary for day-to-day operations of the rehab department
Develop a good rapport with the patient, their family members and third-party payers, and any others involved in the total management of the patient
Qualifications:
Graduate of physical therapy school program approved by the Council of Medical Education of the AMA in collaboration with the APTA or its equivalent.
Licensed, certified, or registered to practice in the State of Illinois or Wisconsin as a physical therapist
Functions according to the APTA Code of Ethics and Standards of Practice
