POSITION SUMMARY: The Community Support Coordinator provides case management services from a strengths-based and goal-oriented perspective with a particular focus on individuals who identify as neurodiverse. The Community Support Coordinator operates restoratively by offering support and accountability to clients facing a variety of challenges, ultimately empowering them to move toward self-sufficiency. The Community Support Coordinator collaborates with many departments including the Counseling Center, Health Center, Disability & Accommodations, and Student Accountability & Conflict Transformation, which together make up the Community Wellness team. The Community Support Coordinator is a non-clinical role.

 

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • In partnership with the Counseling Center Director of Operations and Outreach and the Dean for Community Wellness, develop and coordinate a non-clinical case management program including the development of standard operating procedures.
  • Collaborate with various university departments to provide case management services for a broad range of concerns including Autism Spectrum Disorders and disordered eating.
  • Cultivate and maintain relationships with campus and community resources related to mental health, physical health, housing and financial support, food security, and other social services.
  • Coordinate and implement appropriate interventions and referrals to provide needed support and resources to our community.
  • Serve as a core member of the University’s Behavioral Intervention Team.
  • Leverage internal resources and help build communication plans that support individuals who are having difficulty adjusting to the institution or otherwise navigating institutional systems such as financial aid, student accounts, course registration, housing, disciplinary processes, and taking and returning from a leave of absence.
  • Support individuals who are managing challenging psychological or physical health issues.
  • Collaborate with Counseling Center, Health Center, and Disability & Accommodations, among others, to provide continuity and documentation of care and follow up for significant and emergency emotional and psychological needs (mental and physical health services, financial support, food insecurity, legal services, access to community resources, etc.).
  • Educate members of the community on a variety of social supports and assist with the logistics (such as scheduling, transportation, payment, and insurance) to improve access to those supports.
  • Educate and assist those who are graduating or otherwise leaving the institution in transitioning to long-term support services as needed.
  • Organize internal resources to create access to and awareness about training and workshops for employees and students on a variety of topics including mental health, neurodiversity, social service access, financial need and money management.
  • Consult with faculty, staff, and students who are concerned about another member of the community.
  • Develop and maintain a database of referral resources for social service agencies within the local community and facilitate relationships with off-campus agencies and resources to connect individuals to long-term support.
  • Maintain timely and accurate documentation of all case management activities.
  • Maintain the confidentiality of all records in accordance with FERPA and institutional policy.
  • Ensure the University’s compliance with federal, state, and local regulations related to functional areas.

QUALIFICATIONS:

Master’s degree in social work, counseling, special education or related field with 1-3 years experience providing direct one-on-one service is preferred. Bachelor’s required. Candidates with experience in higher education, familiarity with the Lehigh Valley, and direct experience working with neurodiverse students are preferred. Demonstrated experience working with individuals from marginalized backgrounds including people of color, first generation students, LGBTQ individuals, international students, and individuals with disabilities. Demonstrated commitment to diversity, equity and inclusion. Exceptional organizational and note-taking skills. Demonstrated experience applying the transtheoretical model of behavior change and motivational interviewing techniques.
