JOB SUMMARY:

  • Assumes primary responsibility of fulfilling and implementing the Tobacco Control Program Department’s tobacco cessation workplans as outlined in the ISDH-TPC grant deliverables within the priority area of decreasing Indiana adult smoking rates.
  • Knowledge and use of Center for Disease Control (CDC) Best Practices for Comprehensive Tobacco Control Programs to promote and utilize proven, evidence-based commercial tobacco control prevention and cessation actions and programs to reduce tobacco use and exposure to secondhand smoke in the community.
  • Must complete the Tobacco Treatment Specialist (TTS) Certification within 1 year of hire and participate in required continuing education opportunities to renew certification every 2 years.
  • Engage with all healthcare systems within Hancock County, including Hancock Health, to promote full tobacco cessation health systems change following the Clinical Practice Guidelines for Treating Tobacco use and Dependence.
  • Provides in-person and virtual education and training to healthcare providers, community-based clinics, wellness centers, employers, and other community organizations about tobacco, vaping, and other nicotine addiction, best practices in tobacco cessation treatment protocols, and utilization of the Indiana Tobacco Quitline and the Quit Now Indiana Preferred Network
  • Fulfills the primary responsibility for increasing Hancock County’s engagement with the Indiana Tobacco Quitline (ITQL) through healthcare provider, employer, and other organization referrals and utilization of the ITQL.
  • Develop and build relationships with healthcare providers, community-based clinics, wellness centers, employers, and other community organizations within the Quit Now Indiana Preferred Network monthly to provide follow-up support and recognition of their utilization of the Indiana Tobacco Quitline
  • Engage and build relationships with organizations that work with identified marginalized populations in Hancock County to promote tobacco cessation, the Indiana Tobacco Quitline, Quit Now Indiana Preferred Network, and Coalition membership and/or partnership opportunities.
  • Supports and grows Hancock Health’s continued integration of EMR referrals to the Indiana Tobacco Quitline through Meditech and assists in supporting and training current and future departments and associates upon initial system integration and/or with continued education needs and materials.
  • Promote integration of EMR referrals to the Indiana Tobacco Quitline throughout all healthcare systems and healthcare clinics present in Hancock County to broaden cessation systems change within the healthcare setting.
  • Provides periodic outreach and support to multi-unit housing properties to promote the Indiana Tobacco Quitline for residents and the benefits of smoke-free multi-unit housing in protecting everyone from secondhand smoke. Assist in smoke-free housing policy development and implementation if applicable.
  • Participate in community conversations, presentations, trainings, and meetings with stakeholders to assess needs, engage in advocacy, and measuring quality improvement as it pertains to tobacco cessation systems change and treatment programming in Hancock County
  • Participate in and report to the Hancock County Tobacco Free Coalition during monthly meetings.
  • Represent and promote the Hancock County Tobacco Free Coalition and tobacco cessation resources at community and statewide events, activities, and programs.
  • Contribute to press releases and op eds on local, state, and national developments in tobacco use and tobacco cessation treatment.
  • Submits monthly activity reports in a timely manner to the Tobacco Control Program Director on the data tracking tool provided. Assists with submitting the TPC monthly activity reports through the online tracking tool as requested.
  • Serves on a team to assist with the writing of the biennial Request for Proposal (RFP) grant application to the ISDH-TPC and seeks other grant opportunities to further Coalition capacity in tobacco cessation treatment as needed.
  • Participates in meetings, presentations, conferences, and other continuing education opportunities to support local, statewide, and national commercial tobacco control efforts.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Serve as a team member of the Hancock Health Tobacco Control Program Department
  • Assist with tobacco cessation systems change and best practices in tobacco cessation initiatives at Hancock Health, as well as with other healthcare providers, employers, and other organizations in Hancock County
  • Prioritize organizations that work with identified marginalized populations in Hancock County to promote tobacco cessation, the Indiana Tobacco Quitline, and Coalition membership and/or partnership opportunities.
  • Participate in and report to the Hancock County Tobacco Free Coalition during monthly meetings and represent the Coalition at community and statewide events as requested.
  • Associate can deliver care/services appropriate to patients/customers.
  • Incorporate customer service and quality improvement into all work assignments.
  • Actively seek feedback from each primary customer group served and incorporate changes as needed.
  • Use the PDCA cycle and other quality improvement tools to promote program excellence and cost efficiencies.
  • Maintain relevant certifications, organizational memberships, annual competencies, and participate in professional learning opportunities.
  • Initiate, accepts, and owns other assignments as special needs arise.
  • Demonstrate through words and actions support of departmental goals and objectives.

Position Requirements

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Preferred Bachelor’s Degree in Public Health, Social Work, Counseling, Education, Psychology, or other healthcare field, with at least 2 years previous experience in the field of public health and/or healthcare. Equivalent 5 years tobacco control experience in public health or other healthcare setting may be considered.
  • Must complete the Tobacco Treatment Specialist (TTS) Certification within 1 year of hire and participate in required continuing education opportunities to renew certification every 2 years.
  • Must commit to review and complete the Center for Disease Control (CDC) Best Practices for Comprehensive Tobacco Control Programs in the first 90 days of hire.
  • Must attend other tobacco prevention and cessation program trainings as needed to enhance job knowledge and performance to meet tobacco control trends and community needs.
  • Must complete Hancock Health annual mandatory education including Fire & Safety, Infection Control, Confidentiality, Corporate Compliance, Customer Service, and other required education as assigned.
  • Experience within the healthcare industry, specifically healthcare related teaching experience and working directly with healthcare providers preferred, with a sincere interest and/or experience in the field of tobacco control.
  • Sound understanding of behavior change theory and the practical application of mental health and substance use treatment programming related to tobacco cessation is desirable.
  • Excellent verbal and written communication skills, with public speaking skills a priority
  • Exhibit strong organizational skills and ability to manage multiple projects simultaneously.
  • Experience with building community relationships and working with volunteers is desired.
  • Ability to work both independently and collaboratively .
  • Successful grant-writing experience is desired.
