Leads small, medium and large group activities as well as one-to-one activity programs. Documents participation and completes assessments as applicable. Works with volunteers for the Activity Department. Prepares equipment, supplies and space to accommodate activities and cleans up space after conclusion. Transports residents to and from activities off the neighborhood as needed. Includes field trips and outings. Ensure proper supervision during activities. Make independent decisions when circumstances warrant such action Must maintain appropriate documentation, charting activity participation, behaviors, and degree of involvement. Adhere to facility Policies and Procedures. Work with the Director to create a home like environment in the customer’s room which includes pictures, family photos, plants (please follow dementia guide to toxic plants), afghans, CD player, etc. Ensure customer and family confidentiality regarding all care concerns and issues. Work tactfully with personnel, residents, visitors and the general public Create and maintain an atmosphere of warmth, personal interest, and positivity.
