Description

JOB SUMMARY: Responsible for the day to day supervision of staff working for the Family Reunification Program. Responsible for program's delivery of case management for all clients, providing direct case management services as needed for Level 3 cases. Participates with the Program Director in program planning, staff supervision and training, coordination with Family Reunification oversight and other administrative duties as assigned.

ESSENTIAL FUNCTIONS:

  1. Responsible to ensure efficient and effective dally operations of programs and consult with Program Director as needed.
  2. For all program cases, ensure timely acceptance and submission of electronic case files and reports in funder-approved database (requesting extension via approved protocol as needed) and reviewing and approving all program-level reports prior to submission to funder.
  3. Will provide direct case management services for individual cases as assigned. As Level 3 Case Manager provides and documents wrap around case management services for up to 10 PRS (7 Level 1-2 and 3 Level 3 cases) and up to 3HS (or 13 anytime).
  4. Leverage understanding of current client needs to investigate applicable resources for referral which will include networking with other providers and maintaining a program pool of community resources with contact information and eligibility requirements for staff to referral program clients.
  5. Responsible for the quality of case management services provided to all program cases including the design, implementation, and monitoring of individual service plans, which will include regular case review meetings with program staff to review provided case management services and service plans.
  6. Responsible to review all client records for complete required documentation according to funder, agency, and accreditation standards for all program cases. Will seek case review and/or necessary supervision with Program Director for any cases personally assigned.
  7. Provide mentoring and supervision for all program staff, especially regarding client service issues and barriers making subsequent note of barriers (and workable solutions) to report to funder and/or to request deadline extension on reports.
  8. Prepare staff evaluations, maintain complete personnel folder as per the requirements of funder and monitor training records for all program staff.
  9. Timely composition and submission of monthly program reports to funder under supervision of Director.
  10. Maintain contact with funder requirements via regular meeting attendance with the intention of orienting staff to any/all changes discussed and to report out to funder program's positive outcomes and barriers.
  11. Conduct outreach and provide general community education.

OTHER DUTIES: Willingness and ability to work flexible hours if needed

 
Position Requirements

KNOWLEDGE/SKILLS/ABILITIES: high-level fluency in Spanish with cultural awareness and be knowledgeable about local community social services, family preservation, kinship care, and/or general child welfare.

SUPERVISORY REQUIREMENTS: Must possess proven experience with program management, provision of direct services to diverse populations and knowledge of community resources and entitlement programs. Must demonstrate the ability to work with and supervise others.

MINIMUM QUALIFICATIONS:  LCSW or LPC +2 years specialized experience working with immigrant populations and/or children and families.

LICENSES/CERTIFICATIONS NEEDED: Valid driver’s license.
