Ability to: • Maintain appropriate records, including an admission packet, social history and assessment, and progress notes with a plan of care. • Orient the resident and family to the Health Care Center environment and assist in the adjustment of both, through individual and group programs. • Actively participate in Interdisciplinary Resident Care Plan Conference. • Identify and promote a supportive family or surrogate network. • Establish and maintain an up-to-date and complete resource file on community resources. • Work with staff, residents and family to ensure a smooth discharge transition. • Assist in providing in-services for staff in the area of Social Services. • Provide individual and group help for family and residents at times of adjustment, crisis or particular need. • Provide intense, frequent, short-term therapeutic groups in areas of adjustment re-motivation and self-help skills. • Interpret and promote resident rights and responsibilities. • Assure that residents' personal care needs are met by arranging for shopping and other non-medical personal appointments. • Assist in coordinating psychiatrist visit to ensure compliance with federal and state regulations.   Position Requirements BSW/MSW Experience in long term-care environment.
