Integrated Health Care Liaison

Purpose:

The purpose of Integrated Health Care position is to improve the overall health of Wyandotte County residents by integrating and coordinating medical and behavioral health care within Vibrant Health and Wyandot Behavioral Health Network. This role will be responsible for developing and implementing the National Council’s Comprehensive Health Integration Model between both organization including referral coordination, improving communication and understanding between all entities, connecting to community resources and community education while being client/patient driven.

Accountabilities:

  • Coordinates the identified needs by connecting clients to appropriate supports in the community.
  • Assessing impact of social determinants of health on the person’s ability to access needed healthcare.
  • Develoop the integrated coordination role between WBHN and Vibrant and acknowledging needs and trends building an infrastructure that reduces barriers.
  • Interact positively with other members of both agencies and teams, including active participation in supervision, team meetings, care coordination, continuing education for an understanding of both agencies’ clinical/operational functions.
  • Create a referral system between both agencies that allows for care coordination.
  • Engage individuals and families as partners in services and develop positive, hopeful and trusting relationships.
  • Recognize and understand the emotional distress experienced by clients and their families and guide them to potential resources for identified needs.
  • Demonstrate sensitivity to cultural and ethnic differences.
  • Exhibits exceptional interpersonal skills and ability to work with persons with medical concerns, severe mental illness and/or serious emotional disturbance, adults and youth, and the ability to react effectively in a wide variety of human service situations.
  • Build client relationship, as well as business relationaship and strategic partnerships with other agencies and external resources through outreach activities.
  • Complete all documentation and reporting requirements thoroughly and on time with working knowledge of the two EMR’s between Vibrant Health and WBHN.
  • Regularly travel in and around the region as needed.
  • Carries out the missions for both Vibran Health and WBHN
  • Accepts and uses supervision, direction and consultation to maximize available resources in the completion of work duties.
  • Follow the policies and procedures of both organizations.
  • Additional responsibilities aligned with the scope of work may be assigned as needed.
  • This position will be based at Vibrant Health and WBHN. It is expected that at least 70% of time will be based on-site at Vibrant Health Clinics with regular travel between locations expected.
  • Transportation of clients to other Center locations or at various community sites may be required to meet the business needs of the organization.
 
Position Requirements

Qualifications:

  • Bachelor’s degree or education plus equivalent experience in the human services field, with emphasis preferably in social work, psychology, rehabilitation or related field.
  • Prior experience or knowledge of community mental health and/or community health programs preferred.
  • Knowledge of medical and non-medical services delivery system.
  • Effective communication with members, family and supports, guardians and services providers.
  • Knowledge of trends and issues, laws and regulations related to the development and delivery of community mental health and community health services helpful.
  • Ability to establish relationship with individuals at both organizations and within the social services community.
  • Demonstrated interpersonal and communication skills.
  • Have access to dependable transportation to be used during the work hours.
  • Proof of current Driver’s License (non-suspended and non-expired), automobile insurance and be willing to complete annual vehicle safety inspections.
  • Clearance through the Kansas and Missouri Child Abuse/Neglect Register, Sex Offender background checks, criminal background check, TB testing, pre-employment drug testing and verification of Hepatitis-B immunization.
  • Able to complete the job responsibilities with or without reasonable accommodations.

Work Environment:

  • Frequent exposure to communicable diseases, body fluids, toxic substances, medicinal preparations, radiation and other conditions common to a clinical medical environment may routinely be encountered.
  • Position involves exptensive use of computer, telephone, fax and copier equipment.
  • Prolonged sitting, standing, or walkng may be required.
  • Freqent beding/stooping, squatting, reaching above the shoulders, twisting and turning, kneeling, and pushing/pulling may be required.
  • COVID-19 and the seasonal flu vaccination are required.
