Schedule: 40 hours per week based on the school schedule. The therapist may provide support for afterschool programming, in which case they would work 4, 10hr days instead of 5, 8hr days.

What You Will Be Doing
We want people to join our team who share our values of love, excellence, respect, empowerment, and well-being. The ideal candidate will be responsible for providing outpatient behavioral health services, including counseling and case management services to children, adolescents, and families. Services may be provided in an individual, group, or family basis. This role will work primarily in a school environment but may also be providing services in person or via telehealth, in the office, or in the home/community as needed. The School-Based Therapist reports to the clinical manager and is responsible for ensuring that the treatment needs of the clients are met and maintained to support individual clients psychosocial and emotional needs. Services must be provided in accordance with policies and procedures, and current federal, state, and local standards, guidelines, and regulations.

Primary Job Responsibilities

  • Develops a comprehensive social history and psychosocial assessment that includes the client’s problems, strengths, and preferences, including cultural, religious, and ethnic background and implications for the care plan.
  • Helps clients and families identify needs and develop a service plan to meet those needs.
  • Orients clients and families to the program, its services, service limitations, and clients’ rights.
  • Provide therapy and case management services to address clients’ targeted individualized treatment needs.
  • Involves clients and families in treatment planning, encouraging their attendance at care conferences or with outside providers/professionals.
  • Attends scheduled care conferences and identifies and interprets psychosocial needs of clients for inclusion in the care plan.
  • Completes mandatory documentation within timelines established by applicable regulations and policies.
  • Mediates issues that arise among clients, families, and staff.
  • Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs (e.g., recreational, transportation, healthy/safety, employment, etc).
  • Provides clients and families with crisis management services as needed.
  • Work closely with an interdisciplinary team to support clients and coordinate care.
  • Develops a comprehensive discharge plan in collaboration with clients, clients’ families, multidisciplinary staff, and referral agencies.
  • Trains and consults with staff regarding psychosocial needs that encompass mental health, development, and diversity of individual clients to increase staff knowledge and skills.
  • Communicate by both hearing and speaking in a professional manner with coworkers, outside professionals, and parents, and use professional language when describing the individual and/or behaviors.
  • Responsible for reading and following the employee handbook and all Boundless policies and procedures.
  • Meet required productivity requirements as established by leadership.
  • All other assigned duties. 

Required Education

  • Bachelor’s or master’s degree in social work or related field.

Required Licensure/Certification 

  • Licensed through the State of Ohio as an LSW/LPC/LISW/LPCC/LMFT/IMFT.
  • Ability to obtain and maintain First Aid, CPR - adult, child, and infant. (Training will be provided, if not already certified).
  • Valid Ohio Driver's License with Ohio Bureau of Motor Vehicles; proof of auto insurance; insurable driving record (five points or fewer).

Preferred Education and Experience

  • One to two (1-2) years of supervised social work experience in a behavioral health care setting, working directly with clients.
  • Experience working with people with autism spectrum disorders and developmental disabilities.
  • Experience working with people with mental health conditions.
