Share Email Opening Title Clinician Crossroads   Description Looking for a Full-Time Clinician for our Crossroads to Wellness program Job Summary: You will serve as a member of an interdisciplinary team and fulfill the goals of the Crossroads to Wellness program by providing all needed support services to enrolled consumers based on their goals and desires in WRAP and Recovery Plans. What You Will Do: Provide individual, family, and group counseling services Facilitate psycho-educational groups for consumers on a variety of topics relating to wellness and recovery Be the team lead in completing intake/assessments and formulating Recovery Plans Fulfill case management services, including direct linkages to services, service coordination, and treatment enrollment assistance Communicate observations and information about the consumer in the consumer's service record and to team members Evaluate consumers at intake, admissions, during ongoing recovery, service delivery, and at discharge Collaborate with treatment providers to establish comprehensive assessments and Recovery Plans Create Recovery Plans containing relevant, measurable goals and realistic time frames for achievement Perform reviews of progress and evaluate the effectiveness of goals What We Need to See: Master's Degree in social work or related field from an accredited college or university and experience providing clinical services to adults with serious and persistent mental illness LSW, LAC required, LCSW, LPC preferred A valid driver's license and must be insurable by Agency's automobile insurance What We Offer: 401K, no matching needed High-plan insurance at a generous cost Union benefits Life and Disability insurance Per diem opportunities Excellent supervision and training Continuing education, tuition reimbursement, and eligibility for loan forgiveness 10% daycare discount About CarePlus New Jersey: CPNJ embraces a culture of diversity and inclusion in all of it policies and procedures.
