Case Manager ICMS

Looking to hire Case Managers for our Integrated Case Management Services Program


Job Summary:

You will provide direct case management services to an adult mental health population. You will spend 50% of your time face-to-face with consumers in their homes and the community.


What You Will Do:

  • Facilitate and coordinate psycho-educational services for consumers and their family members on a variety of topics relating to wellness, health promotion, prevention, skill-building, and recovery topics
  • Offer on-site and outreach services (including transport of consumers)
  • Fulfill case management services, including coordinating or directly facilitating linkages to services, service coordination, and treatment enrollment assistance
  • Ensure consumers have community support in place to address identified needs
  • Attend routine supervision and team meetings to review cases and discuss program operations
  • Participate in supportive counseling and crisis intervention
  • Participate in the assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery, and at discharge
  • Conduct case management intake assessments and formulate plans of care
  • Perform reviews of progress and evaluate the effectiveness of goals
  • Work with administrative staff to ensure that all services are documented based on billing and regulatory standards

 

What We Need to See:

  • Master’s Degree in a Mental Health-related field OR
  • Bachelor's Degree in a Mental Health-related field, plus one (1) year post-BA experience in the Mental Health field
  • Valid driver's license insurable by agency's automobile insurance
