Looking for 2 full-time Visitation Coordination and Reunification Clinicians. Provisionally Licensed Clinician's starting at $60,000!


Job Summary:

You will provide outpatient therapeutic sessions to children and adolescents and their families to increase family stabilization, maintain accurate and timely records for each case, and ensure departmental goals or objectives are accomplished within the prescribed time frame by performing the job responsibilities personally.


What You Will Do:

  • Assess and evaluate clients at intake and/or admission, during ongoing treatment and/or service delivery, and at discharge and/or transfer
  • Formulate psychosocial treatment plans with relevant goals, behaviorally measurable objectives that serve as indexes for evaluating treatment progress, and realistic time frames for achievement.
  • Provide services and supportive interventions, including case management, linkage to community resources, and provides inter-agency coordination of treatment.
  • Document services (i.e. assessments, treatment plans, progress reports, and discharge summaries) in clinical records on a timely basis with all entries authenticated and time/dated
  • Plan for, communicates, and documents discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum
  • Function as a member of an interdisciplinary team in the assessment and/or evaluation of clients, articulate in written documentation the integration of client history and assessment information and actively participate in program and clinical team meetings, provide support for team colleagues, and engage in program-related performance improvement processes and activities.
     

What We Need to See:

  • Master’s degree in a behavioral health discipline from an accredited college or university and documented knowledge of
  • At least three (3) years of work experience with children and adolescent populations.
  • Prof. License as required by program’s requirements (e.g.: LAC and/or LSW)
  • A valid driver's license and must be insurable by agency's automobile insurance
  • One position Bilingual (Spanish) required; one position Bilingual preferred
