Since 1971, Hampton-Newport News Community Services Board has provided a comprehensive continuum of high-quality, essential, and community-based services to citizens of the Peninsula who are affected by mental illness, developmental disabilities, and substance use disorders.
Support Coordinationassists individuals with developmental disabilities and their families in accessing services, supports, and resources within their community, utilizing a person-centered approach. Support Coordinationfunctions as the entry to access needed community resources and services.
The Support Coordinator ensures continuity of directly provided and contractual services to agency's individuals with intellectual and developmental disabilities. Support Coordinators assist individuals with their basic physical, financial, social and personal needs. Support Coordination services for the individuals include development of person centered plans, service provider linkage, monitoring, advocacy, counseling crisis intervention, daily living support, and enhancing community integration.

Requirements: Bachelor's degree in Human Services (Social Work, Psychology or Counseling) with one (1) year experience in support coordination services with the assigned population. Valid Driver's License.
The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
