Schedule: Monday - Friday 8a - 4:30p and one weekend every 2 months
Since 1971, Hampton-Newport News Community Services Board has provided a comprehensive continuum of high-quality, essential, and community-based services to citizens of the Peninsula who are affected by mental illness, developmental disabilities, and substance use disorders.
The ACT program provides comprehensive community based treatment services to persons with severe and persistent mental illness in Hampton or Newport News who have been unable to make use of traditional outpatient mental health services.
The Service Coordinator ensures continuity of directly provided and contractual services to agency's mentally ill, with possible co-occurring intellectual developmental disabilities and substance abusing client populations. Service Coordinators assist clients with their basic physical, financial, social and personal needs. Service coordination services for the client include outreach, psychological and need assessments, treatment planning and service provider linkage, monitoring, advocacy, counseling and crisis intervention, daily living support, medication supervision, and similar functions.

Requirements:
Bachelors degree in Human Services (Social Work, Psychology, or Counseling) with one (1) year experience in case management or service coordination services. Master's degree preferred. Must be registered with the Board of Counseling to practice as a QMHP-A or register with the Board of Counseling to practice as a QMHP-A trainee within the first 30 days of employment. Valid Driver's Licenses
The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
