Wyandot BHN is seeking a dedicated Community Resource Navigator (CRN) to join our team! The CRN plays a central role in the Greater Kansas City Coordinated Entry System working with households experiencing homelessness or chronic homelessness, building an early rapport and supporting them to the point where they are able to secure permanent housing. CES Navigator will work with caseload of potential participants, to ensure connections to support services, securing eligibility documentation and referring them to available housing programs. In partnership and collaboration with social service and housing providers, homeless households will decrease the time between referral and being housed. The CRN works closely with other CRNs in the Kansas City region to help shape and improve the Greater Kansas City CES.
Qualifications:
Prior experience working with diverse populations, specifically with adults with severe and persistent mental illness, people with substance use disorders, and people who are homeless or formerly homeless.
Lived experience with homelessness is not mandatory but will be considered an advantage.
Ability to establish relationships with individuals and organizations of influence within the social services community, including civic groups, charitable agencies, related government entities, fundraising sources, etc.
Essential Job Duties:
Conducts common assessment tool and triages households experiencing homelessness or a housing crisis.
Communicates with CES participants that have completed the common assessment tool.
Identifies appropriate interventions to address and resolve the housing crisis.
Receives, assesses and triages participants from CES navigators and community partners.
Assists households with collecting documents necessary for permanent housing.
Assists households with accessing permanent housing opportunities including Permanent Supportive Housing, Rapid Re-Housing, other subsidized housing, and affordable housing.
Manages and tracks new referrals and ensures timely placements.
Connects households to supportive services as needed such as mental health, health, substance abuse, employment, etc.
Conducts outreach activities to engage chronically homeless individuals toward pursuing permanent housing options
Input and review data in the Homeless Management Information System (HMIS) for accuracy, completeness and updating information accordingly.
Documents services and completes data entry into HMIS database.
Attends phone conferences, workgroups and case conferencing meetings to coordinate services with other providers
Coordinate with program managers and direct care staff from various agencies and programs that utilize the Greater Kansas City Coordinated Entry System
Provide additional support necessary to finalize the housing placement and provide continuity to people who are newly housed to stabilize placement and ensure connection in the community.
Work closely with housing staff to identify housing options for households based on their preference and eligibility.
Responsible for gathering and compiling participant/service/outreach data and preparing timely periodic reports, as required by funders and collaborative partners
Maintains confidentiality of all privileged information
Work collaboratively to develop program plan to target diversion and prevention assistance for the Greater Kansas City Coordinated Entry System.
Track outcomes as directed by the Greater Kansas City Coalition to End Homelessness and by the agency
