Description

JOB PURPOSE:

The Nurse Case Manager will provide comprehensive medical case management to active clients enrolled in medication management at the Cohen Clinics at Endeavors. Services will be a mixture of virtual and in-person. The Nurse Case Manager will offer direct support to the Regional Prescriber and to clients who are referred for medication management.

 
Position Requirements

ESSENTIAL JOB RESPONSIBILITIES:

  • Direct the referral process for medication management clients who are eligible for psychiatry care. To include ensuring all relevant consents, medical history information, referral paperwork, and release of information are completed to ensure collaborative and comprehensive psychiatry care can occur.
  • Conduct initial administrative and medical support tasks for all clients scheduled for the psychiatry care team. Conduct in-person vitals, uranalysis drug screens, and document results in the client’s electronic health record. Collect any pre-visit requirements for each client and document this information in the electronic health record (lab work, vitals, drug screens, etc.).
  • Provide clients with appropriate referrals to medical care, lab work, or other access to care needs to support the health needs of the clients. Support clients being enrolled in insurance, applying for benefits, and connecting with appropriate health care options.
  • Meet regularly with the Psychiatry care team to review caseload. Work closely with the clinical staff for referrals and scheduling for psychiatry clients.
  • Demonstrate exceptional customer service, in everything you do, by placing the child, family, Veteran or client first to support our mission to “Empower people to build better lives for themselves, their families and their communities.”

 

ESSENTIAL QUALIFICATIONS:

EDUCATION: Graduate of an accredited school of Nursing or completion of an approved program is required. Associates’ Degree in Nursing is preferred.

EXPERIENCE: 1+ year-experience required in the case management field or clinical patient setting, nursing, or relevant in a medical related field.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer’s sole discretion.

LICENSES: Active Licensed Vocational Nurse, Driver’s License, Basic Life Support (BLS) Certification

VEHICLE: Must have daily use of a vehicle without prior notice.

TRAVEL: This position will support the psychiatry care team in San Antonio, Killeen, and El Paso. Business related travel possible, but rare.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.

Preferential hiring consideration given to Reservists, Veterans, or military spouses.
