The ACT Nurse/Case Manager will provide intensive case management, rehabilitation, and supportive services to individuals experiencing serious mental illness. Benefits Sign-on bonus of $1,500.00 included with the first paycheck. Health insurance as low as $20.00 per month for individuals. Family and dependent plans are also available. Nine paid holidays. Accrue up to 20 days of paid time off (PTO) per year from year one. PTO is immediately available upon accrual. Participation in the Virginia Retirement System. Professional development assistance provided at no cost. Summary of Essential Functions Serves as primary provider of Assertive Community Treatment (ACT). Provides nursing and case management services to individuals experiencing chronic mental illness. Assists individuals in maintaining independent living. Meets requirements for documentation of client service delivery and maintains records in accordance with agency and regulatory standards. Provides clinical and program consultation to individuals, organizations, agencies, and groups that are involved with VCSB. Participates in team meetings, clinical staffing, and supervision. Participates in in-service training/staff development activities regarding the operation of ACT services. Minimum Qualifications All applicants must meet the following minimum qualifications in order to be considered qualified for the position: Associate’s Degree in Human Services, Active RN or LPN license, QPPMH, QMHP, or QMHP-T eligible, One year of clinical experience working with adults experiencing chronic mental illness, and Valid Virginia driver’s license with acceptable driving record   Position Requirements   Salary Information Salary is negotiable based on relevant experience.
