As a member of the highly collaborative Community-Based Services team, the Child Mental Health Case Manager will play a vital role in providing and connecting children and their families to support services. Benefits Sign-on bonus of $1,500, included in full with the first paycheck. Flexible work schedule and environment. Incentive pay available. Accrue up to 20 days of PTO per year, which is immediately available upon accrual. Summary of Essential Functions Provide intensive case management services to children and adolescents at risk of or diagnosed as severely emotionally disturbed. Collaborate with the children, adolescents, and their families to facilitate adjustment at home, in school, and in the community. Help children, adolescents, and their families navigate crisis stabilization and emergency intervention services as needed. Develop and complete Individual Service Plan (ISP) reviews on caseload in accordance with agency standards and Medicaid criteria. Coordinate care and discharge planning with the Commonwealth Center for Children and Adolescents (CCCA). Minimum Requirements Bachelor’s degree in human services or related field from an accredited program. At least one year of experience working with children or adolescents in a clinical mental health setting. Must pass required pre-employment screenings, including, but not limited to, criminal history and experience verification.   Position Requirements   Salary Information Up to $51,301 per year, commensurate with education and experience.   EOE Statement Valley Community Services Board is an equal employment opportunity employer.
