Valley Community Services Board serves as the single point of entry into publicly funded mental health, developmental, and substance use disorder services for these localities.

Description

VCSB is looking for a Critical Time Intervention (CTI) Case Manager (CM)! The CTI CM will provide intensive, time-limited, community mental health case management to Permanent Supportive Housing (PSH) participants who are transitioning from hospitalization or homelessness to permanent housing.

Summary of Essential Functions

  • Provides comprehensive initial and annual assessment of client strengths and needs.
  • Participates in development of the consumer service plan.
  • Refers, links, and coordinates Medicaid and non-Medicaid services.
  • Advocates and promotes recovery oriented practice.
  • Provides counseling and case management services.
  • Provides crisis intervention and crisis stabilization services as needed.

Minimum Qualifications

All applicants must meet the following minimum qualifications in order to be considered qualified for the position:

  • One year of clinical experience with adults experiencing serious mental illness, including developing, conducting, and approving assessments and individual service/treatment plans, and
  • Valid Virginia driver’s license with an acceptable driving record.
