Since 1971, Hampton-Newport News Community Services Board has provided a comprehensive continuum of high-quality, essential, and community-based services to citizens of the Peninsula who are affected by mental illness, developmental disabilities, and substance use disorders.
Intensive Care Coordination (ICC) provides a more intensive level of case management services including clinical assessments and level of care determinations for those with a diagnosis of Serious Emotional Disturbance (SED) or Intellectual or Developmental Disability (IDD).
The Intenseive Care Coordination Case Manager insures continuity of directly provided and contractual services to agency’s mental health, developmental disability, and substance use disorder client populations. Intensive Care Coordinators provide comprehensive assessment; treatment planning, direction, monitoring and coordination; referral and follow up; discharge planning; and quality assurance for Children’s Services Act funded moderate to severely emotionally disturbed children and their families. Insures that assigned children receive the services that comply with service decision and level of care criteria; monitors service provider’s attainment of treatment specific goals; authorizes level of care and provider changes when indicated. Promotes the meaningful participation of family members, community supports, and stakeholder agencies in crafting innovative service plans, identifying and mobilizing resources, providing services, supervising clients and monitoring outcomes. Insures that assigned children are served in the least restrictive and least intensive level of care that meets their identified needs, focusing on children living in family-based or community settings. Activities include the facilitation of a team approach, outreach, advocacy, counseling and crisis intervention, daily living support, and similar functions.

Requirements: Bachelor’s degree in Human Services (Social Work, Psychology, or Counseling) with at least two years of experience providing children’s mental health care management services. Knowledge and experience with DSM V diagnosing preferred. Valid Drivers' Licenses.
The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
