The Case Manager is integral to the Community Schools team, committed to serving, supporting, and strengthening the community. The Case Manager will work collaboratively with schools, students, and families to provide support to students and families experiencing socioeconomic or health stressors. The Case Manager will work primarily from the school but will also be responsible for conducting regular home visits and supporting families within the community. In addition, the Case Manager will assist the team with coordinating and implementing monthly family forums.   Position Requirements BSW with two or more years of experience in social services, specifically focusing on serving children and families.
