SUMMARY: The OB Health Educator works as a Comprehensive Perinatal Health Worker for Open Door Community Health Centers (ODCHC) provides OB and early infant care health education in alignment with the Comprehensive Prenatal Services Program (CPSP). The Health Educator is responsible for informing perinatal patients about their own health and wellness and the health and wellness of their infant and connecting patients with resources for maintaining their health in the future.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Completes special projects as related to CPSP program.
  • Provides perinatal community outreach work as assigned.
  • Assists with Medi-cal applications for presumptive eligibility.
  • Meets minimum requirements for 20 client individually billable encounters per week (1 FTE).
  • Exhibits independence and initiative in the development and implementation of perinatal group, individual education, support visits and patient materials.
  • Supports care team with disability paperwork, patient letters, and chart updating.
  • Maintains appropriate client follow-up and tracking.
  • Actively participates in monthly high risk OB meetings where development and evaluation of progress in the implementation of Individual Care Plans (ICP).

 

PATIENT EDUCATION, INTERVENTIONS AND REFERRALS:

  • Counsels and educates pregnant and postpartum clients on pregnancy, childbirth, and postpartum care, perinatal case management, and support services.
  • Develops, updates, organizes, and implements ICPs for each OB patient in caseload (includes pregnancy and first six weeks postpartum) under the direction of a medical provider.
  • Assesses nutrition, psychosocial, and health education needs for OB patients.
  • Identifies and records strengths, problems, and high-risk conditions on the ICP.
  • Prioritizes patients’ needs, strengths, and problems on the ICP.
  • Proposes solutions based on assessments, individual needs, and cultural needs of the patient.
  • Modifies ICP as problems develop, improve, or resolve.
  • Ensures that each patient is offered the minimum CPSP visits: Orientation and periodic (first trimester, second trimester, third trimester, and postpartum).
  • Orders initial labs during orientation per standing order.
  • Assists the patient and providers in fulfilling recommendations for plan of care, which may include: Genetic Counseling, Public Health Nursing home visits, FNP Program, Dental, WIC, etc.
  • Making or facilitating new and follow-up appointments within and outside of ODCHC.
  • Follow-up of “no show” appointments.
  • Provides and/or explains special instructions, transportation arrangements, assistance with translation needs, etc.
  • Provides ongoing assessment of the patient’s health status and appropriateness of interventions, as well as results from any interventions.
  • Maintains communication and close contact with each patient throughout pregnancy and postpartum period patients.
  • Other duties and responsibilities as designated by supervisor.

 

QUALIFICATIONS AND EXPECTATIONS:

  • Strong interpersonal skills and the ability to establish constructive professional working relationships among a diverse workforce.
  • Ability to work as a member of a team and independently as needed.
  • Ability to handle difficult or confrontational situations in a calm, consistent, and equitable manner.
  • Ability to write routine reports and correspondence.
  • Ability to effectively represent ODCHC’s interests in the community and maintain effective working relationships among co-workers, public, private, and professional groups.
  • Strong computer skills with specific aptitude in Microsoft Office Suite and/or Electronic Medical Records (EMR).
  • Fluency in English and Spanish (speak, read, and write) preferred.
  • Adheres to the scope of practice of a medical assistant as determined by the California board of medicine. (If no MA certification we can train and get you certified.)
  • Adheres to ODCHC’s policies and procedures.

 

EDUCATION AND EXPERIENCE:

  • High school graduate or equivalent
  • Minimum 1 year paid experience in prenatal and/or postpartum care.

 

CERTIFICATES, LICENSES, REGISTRATIONS:

  • Lactation Educator preferred.
  • Medical Assistant Certification or Credentialing.

 

SUPERVISORY RESPONSIBILITIES: None.

 

SUPERVISION AND SUPPORT: The OB Health Educator reports directly to the Registered Nurse Clinic Manager or Back Office Manager and is an integral member of health center team.

 

PHYSICAL REQUIREMENTS: This is largely an office-based position. The physical requirements described are representative of those needed to successfully perform the essential duties of the position. Reasonable accommodation will be made to allow otherwise qualified candidates to perform these functions.

 

  • Ability to communicate via telephone, video, and/or in-person.
  • Vision adequate to read documents, computer screens, forms.
  • Ability to remain stationary for extended periods.
  • Ability to lift, carry, or otherwise move up to 25 pounds.
  • Ability to use keyboard and view computer screens for extended periods.
  • Ability to travel locally and long-distance as needed.
  • Ability to move around offices and clinics as needed.

 

Hiring Range $23.70 to $29.00
