Schedule: This is a part-time position, 16-24 hours per week, and the hours of work and days are Monday through Friday. Occasional evening and weekend work may be required as job duties demand.

Pay Range: $23.30 - $26.00 an hour based on education, experience, and credentials.

What You Will Be Doing
We want people to join our team who share our values of love, excellence, respect, empowerment, and well-being. The ideal candidate will be providing nursing and behavioral health services to individuals served within the community including in our supported living and behavioral health services. Services may be provided on an individual, or family basis in the office, via telehealth, in clients’ school or home environments, or in other settings within the community. The LPN reports to Community Nurse Supervisor, and Statewide DON, and is responsible for working within the scope of their license to ensure that the physical health and behavioral health needs of the clients are met and maintained on an individualized basis in accordance with policies and procedures, and current federal, state, and local standards, guidelines, and regulations.

Primary Job Responsibilities
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.  

  • Acting on the direction of a licensed physician, licensed dentist, physician assistant, podiatrist, optometrist, or chiropractor, or at the direction of a registered nurse, administers prescribed medications/treatments; provides oversight and maintains medication accountability records; maintains medication equipment and supplies; performs skilled nursing procedures (e.g., applies and changes dressings, catheterizes); assists in medical procedures; requests, secures and follows up on medical information of individuals.  
  • Provides general nursing care to individuals; maintains first-aid supplies and provides first-aid treatment as necessary; screens for communicable and nuisance diseases.  
  • Observes and documents conditions and responses to, and progress of, nursing care treatment and medications; reports unusual developments to the supervisor or health care provider.  
  • Provides education and training to individuals/caregivers/guardians/staff in health-related matters (e.g., nutrition, symptoms, hygiene, safety), as applicable to behavioral health/developmental needs.
  • Prepares and maintains appropriate documentation of service delivery and reports as required; ensures compliance with all reporting standards.
  • Accepts on-call rotation as scheduled. 
  • Participates as a member of the interdisciplinary team working with other professional staff by contributing to planning, implementing, and evaluating nursing care plans to best support the needs of the individuals.
  • Provides consultation services to staff, individuals, parents, and significant others regarding health and safety-related issues, as directed by the RN or physician, physician assistant, dentist, podiatrist, optometrist, or chiropractor.
  • Gather health information for individuals served including screenings, assessments, and vital signs.
  • Monitor individuals’ symptoms and effects of medications and share/report this information with appropriate team members, as applicable.
  • Attends medical appointments with individuals served as needed.
  • Meets specified productivity standards.
  • Obtain vital signs and triage individuals in outpatient office setting for psychiatry and primary care as needed.
  • Assist the individual and staff as applicable in self-care techniques (to curb behaviors and limit medication use, etc.)
  • Manage behavioral crisis situations and provide interventions as needed.
  • Identify and report any environmental or safety concerns to the supervisor, team, and appropriate reporting body as applicable.
  • Participation in departmental meetings, committee meetings, and others as requested.
  • Performs other related duties as assigned.  

Minimum Education/Qualifications

  • Must be at least 18 years of age.
  • Must be able to communicate in English, including both spoken and in writing. 

Required Licensure/Certification

  • Current and in good standing LPN License with the Ohio State Board of Nursing. 
  • First Aid, CPR - adult, child, and infant; or the need the ability to become certified in CPR/FA within 60 days of hire.  
  • Required valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles; proof of auto insurance; insurable driving record (five points or fewer).
  • Successful completion of training in trauma-responsive care and crisis de-escalation/management within 60 days of hire. 
  • Completion of medication delegation training.

Preferred Education/Experience

  • One year of experience working as an LPN with individuals with behavioral health conditions or intellectual/ developmental disabilities.
