Responsible for aftercare planning and aftercare Groups for all patients assigned to their caseload. Meet in a timely manner with each patient to develop an aftercare plan. Conduct aftercare groups focusing on identifying aftercare needs, relapse, and aftercare planning options. Provide family services when needed through family counseling and/or educational group modalities. Participate fully in Team Treatment meetings, Unit meetings, and Program meetings. Assist patients in maintaining contact with external systems where necessary regarding Probation or Parole Officer, DPA, DHS, employer's funding resources, etc. Maintain all appropriate documentation and correspondence relative to each patient's individual medical record, according to Hospital, State, and Federal Standards. Completes Informed Consent for each contact person/agency as needed and documents properly.   Position Requirements PA LSW with work experience related to the field of substance dependence treatment is desirable.   This position is currently not accepting applications.
