Looking for a full-time counselor for our Family First program.

Job Summary:
You will provide in-home counseling, on-call/in-person crisis intervention, and case management to DCPP-referred, high-risk children and their families to prevent abuse/neglect. Assures departmental goals or objectives are accomplished within the prescribed time frame by performing the job responsibilities personally.

What You Will Do:

  • Provide in-home services and supportive interventions including case management, linkage to community resources, and provides inter-agency coordination of treatment
  • Conduct assessment/evaluation of clients at intake, during ongoing service, delivery, and at termination
  • Formulate relevant goals with behaviorally measurable objectives that serve as indices for evaluating intervention progress and realistic time frames for achievement
  • Prepare written documentation of assessments, interception plans, progress notes, and discharge summaries in the clients' files with all entries authenticated and time/dated
  • Plan for, communicates, and document aftercare plans for clients completing program and/or accessing other services in the continuum.
  • Function within an interdisciplinary team in the assessment/evaluation of clients, and to articulate verbally and in written documentation the integration if client history and assessment information
  • Collaborate as a member of an interdisciplinary team, including but not limited to, active participation in program and team meetings, providing support to team colleagues, and engaging in program- related performance improvement processes and activities and providing leadership within team and functions
  • Participate in weekly team case conferences and trainings as per Family Preservation Services' requirements
  • Provide 24/7 coverage to caseload


What We Need to See:

  • Bachelor's degree or a Master's degree in Social Work OR another behavioral health discipline from an accredited college or university
  • Two (2) years of experience providing counseling to children and families
  • A valid driver's license and must be insurable by agency's automobile insurance.
