Join our team as a Community Health Worker for HUB, La Crosse

 

As a Community Health Worker, you help to bridge the gap between health care delivery and the social service sector for cost savings, improved population health outcomes, and increased client experience and engagement.

 

Community Health Workers identify clients risks, identify outcomes to achieve (Pathways) to reduce their risk and navigates the client through the various health systems to obtain the services they need. This helps to streamline the acquisition of services for clients and improves communication to service providers for more effective care.

 

What are the primary responsibilities of this Community Health Worker position?

  • Focus on Covid Resilience, specifically in populations that are at risk for severe Covid.
  • Provide information and resources to clients on Covid and Covid vaccinations
  • Provide social, cultural, behavioral, and resource support to clients as they navigate the health care and social service systems.

Experience Preferred:

  • Minimum of one-year experience working or volunteering in human services, healthcare, community outreach, health education, or related direct service
  • Knowledge of local resource agencies and resources available

 

Education

  • Some college preferred.
