Under the supervision of physician, obtains medical histories, performs physical examinations, makes diagnosis and orders appropriate and standard laboratory and diagnostic procedures as necessary in the office. Evaluates procedure results and finding to physicians. Provides continuity of care by developing and implementing patient’s plan of care. Provides patient education about underlying medical condition, self-management of acute or chronic illnesses and tailors instructions to patients’ individual circumstances. Instructs and counsels patients regarding therapeutic regimens, promoting wellness and health maintenance. Prescribes medications. Administers medication/immunizations, suturing, managing wounds and infections, etc. Documents exam and progress notes. Provides referral letters as needed. In addition to office visits, the Nurse Practitioner/Physician Assistant will perform other tests and procedures specific to the specialty. Collects, monitors and reports on patient outcomes. Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill patients. Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs. Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities. Implements interventions to support the patient to regain or maintain physiologic stability, including but not limited to serving in the first assisting role. Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring and discharging patients. Collaborates with multidisciplinary team members by making appropriate referrals. Facilitates staff, patient and family decision making by providing educational tools. Other duties as assigned by supervising physicians. Certification as an Adult or Family Nurse Practitioner or Physician Assistant. PREFERRED QUALIFICATIONS: Ability to work with other medical professionals. EMR experience. In accordance with the New York State Department of Health (DOH) order that all hospitals and nursing homes “continuously require all covered personnel to be fully vaccinated against COVID-19.”  Upon receipt of a conditional job offer, candidates who are not already vaccinated must obtain the first dose of the vaccine within three (3) calendar days of job offer and must obtain any subsequent doses in accordance with the vaccine protocol.
