Specific Job Detail

General Responsibilities

  • Coordinate and manage all day-to-day business affairs and other operational activities related to the administration of the palliative care department.
  • Answer the phone within three (3) rings. Record the outcome of calls either manually or in an automated manner, make follow-up calls as necessary, and identify/refer to alternative resources when applicable.
  • Assist Central Intake Admission Coordinator with obtaining insurance pre-authorizations and initiating patient contact.
  • Ensures the maintenance of up-to-date account information for existing referral sources (i.e. address, contact, preferences, etc.) and maintains referral source communication preference information.
  • Maintains insurance records on all patients and obtains timely re-authorizations for continued care when necessary. Demonstrates understanding of all payer sources and impact on revenue.
  • Advise Palliative Care Team of patient acceptance and provide all relevant information including accurate insurance and admission information in order to ensure appropriate disclosure of rights and responsibilities, financial liabilities and benefit plan to patients and their family.
  • Maintain communication with pending patients, their families and referral sources and routinely follows the progress of prospective patients in order to assure continuity of care.
  • Manage all daily clinical records functions including implementing clinical records policies. Ensure clinical record systems are maintained in compliance with state and federal regulations and company policies.
  • Assist as requested to provide timely responses and further information related to all billing denials, and postpayments audits of submitted claims.
  • Establish and maintain professional and productive relationships with all referral sources. Acts as a resource for customers and community as a whole regarding palliative care.
  • Maintain standard of high-quality customer service and monitor and track referral sources' satisfaction levels. Identify opportunities for additional or improved services to address unmet customer needs. Communicates customer service issue to Administrator as appropriate.
  • Attend daily hospice morning stand-up meeting prepared to discuss palliative census, pending admissions, discharges and issues or concerns.
  • Track and maintain medical and office supplies and reorder as requested.
  • Demonstrate working knowledge of computer systems.
  • Assist with coordination and scheduling of palliative and hospice personnel, as appropriate.
  • Produce census list, bi-weekly IDT list, and other lists as required.
  • Print reports as required off computer.
  • Mail patient satisfaction surveys, tracks their return and reports results to Administrator.
  • Process discharged charts and breakdown to close files.
  • File and maintain palliative policy manuals and all other master manuals.
  • Apply the policies and procedures of the palliative program and the rules and regulations of state and federal regulatory agencies and other certified agencies in providing care.
  • Willingly accepts direction from the Administrator, Director of Palliative Care Services, Director of Operations, Director of Clinical Services.
  • Participate in orientation of new staff if requested.
  • Adheres to established personnel policies.
  • Ensure growth and profitability of the company through the responsible use of company resources.
  • Maintain a professional and quality image at all times.
  • Accepts other assignments as deemed appropriate.

ProMedica Senior Care, formerly HCR ManorCare, provides a range of services including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.

The Pallative Care Administrative Coordinator is responsible for coordinating and managing all day-to-day business affairs and other operational activities related to the administrative of the palliative care department. Educational Requirements High school diploma or equivalent

Experience

  • Prior experience in home health, hospice or palliative care environment preferred

 

Travel

  • Less than 5%.
