Specific Job Detail

General Responsibilities

  • Develops a bereavement program encompassing 13 months for the hospice patient's caregiver and/or other designated survivors
  • Consults with the Volunteer Coordinator regarding assignment and supervision of the bereavement volunteers
  • Participates in orientation, training and evaluation of bereavement volunteers
  • Develops programs for bereavement group activities
  • Develops resource of written material, videos, and referral sources for caregivers and the interdisciplinary team
  • Provides bereavement follow-up with patient care staff and volunteers
  • Participates in regularly scheduled interdisciplinary team meetings to coordinate bereavement care, exchange information and problem solve, and to provide staff support and in-service education
  • Prepares reports as requested by the Director of Professional Services, Patient Care Coordinator and agency policies
  • Participates in programs providing staff orientation, training and education
  • Participates in agency quality assessment performance improvement (QAPI) projects
  • Responsible for tracking of bereavement services for each patient's primary caregiver for 13 months
  • Participates in community programs on hospice and related issues of death and bereavement, including providing in-services
  • Accepts other assignments deemed appropriate.
  • Provides bereavement and counseling services to hospice patients' caregiver and/or significant other per bereavement plan
  • Directs or performs grief assessment of bereaved caregiver members
  • Develops and implements the Bereavement Plan of Care based on assessment and consultation with team members. Updates plan as changes occur
  • Assists patient, caregiver & interdisciplinary team in acquiring & utilizing community bereavement resources as needed in dealing with bereavement issues
  • Completes bereavement paperwork and care plans
  • Maintains patient records and documentation of activity to ensure continuity of care
  • Provides grief education and support to patient/caregivers and interdisciplinary team. Offers services to community at large
  • Maintains current knowledge of bereavement industry through participation in continuing education programs, workshops and professional organizations
  • Participates in on-call rotation as appropriate
  • Willingly accepts direction from Director of Professional Services and/or Patient Care Coordinator
  • Understanding of third party payer requirements and the Hospice Medicare Benefit for payment of services/products
  • Schedules visits with patient/caregiver and confirms the appointment as needed
  • Visits are scheduled according to patient/caregiver need
  • May make visits to patient/caregiver prior to death when requested
  • Is available to staff for bereavement support
  • Maintains confidentiality
  • Insures that the caregiver's choices regarding bereavement contact or services are honored
  • Evaluates and assesses grief needs and risk factors associated with grief
  • Assesses caregiver response to grief and loss issues
  • Develops a bereavement plan of care, reflecting caregiver needs and frequency of services

Description

ProMedica Senior Care, formerly HCR ManorCare, provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.

The Bereavement Coordinator is responsible to develop, implement and supervise the hospice bereavement program. Provides bereavement support through grief support, memorial services, family assessment and conference. Responsible for consultation to interdisciplinary team, client and family in issues related to grief and loss.

Educational Requirements Bachelor's degree in counseling, theology or related field required; Master's degree in the area of mental health preferred.  
Position Requirements

Qualifications

  • Ability to develop and maintain rapport with caregiver and patients.
  • Excellent communication skills, both verbal and written.
  • Ability to work well with an interdisciplinary team.
  • Excellent skills in interpersonal relations and organization.
  • Demonstrated problem solving and decision making abilities.
  • Demonstrated knowledge of bereavement concepts; ability to relate concepts to personal experience.
  • Knowledge of community resources and methods for patient/caregiver referrals.

 

Experience

  • Experience in caregiver support and counseling regarding issues of loss, grief, death and dying.
  • Experience in developing and conducting programs of support.
  • One year experience in home health care or hospice preferred.
  • Proficient computer skills preferred.

 

Travel

  • Necessary on a daily basis.

 

Working Conditions

  • Exposure to disagreeable conditions including odors, infection, illness, and physical contact from disruptive patients.
