Hospital Liaison & AOT Coordinator  

Description

Wyandot BHN is seeking a dedicated, full time State Hospital Liaison to provide liaison services to Wyandotte County adult residents hospitalized at Osawatomie State Hospital (OSH) or designative state hospital alternative beds. Participate in admission, ongoing treatment, AOT, and discharge planning. Coordinate follow-up services to ensure a smooth transition from the hospital back to community based services. This position reports to the Manager of Crisis and Access Services.

Accountabilities:

  • Travel weekly to Osawatomie State Hospital and other facilities as appropriate.
  • Coordinate information and services between Wyandot Center and OSH or designated state hospital alternative beds that relate to a consumers admission, ongoing treatment, discharge and outpatient follow-up.
  • Coordinate intakes to open services at Wyandot Center for clients at OSH or other hospitals as needed.
  • Serve as a representative of Wyandot Center at OSH and as a resource for community based services.
  • Confer with Wyandot Center staff, family and professionals from other agencies to gather information, coordinate services and participate in discharge planning.
  • Identify clients that would benefit from Suicide Care Pathway and make appropriate referral
  • Attend treatment team meetings at OSH for Wyandotte county residents as allowed by OSH.
  • Actively participate in the treatment planning of consumers who are admitted to OSH or designated state hospital alternatives by seeking input from treatment team members from both Wyandot Center and the hospital, participating in discharge planning and coordinating services upon discharge.
  • Responsible for requesting and securing appropriate state hospital records to be included in Wyandot Center’s medical record(s).
  • Maintain program data and develop reports to insure appropriate accountability.
  • Complete required documentation in a timely manner.
  • Attend quarterly liaison meetings at OSH.
  • Maintain positive and professional working relationships with all staff at OSH, other psychiatric hospital units and Wyandot Center and seeks out supervision/support when conflict arises.
  • Collaborate with OSH and nursing facility for mental health staff, to assist with obtaining nursing home placement, as needed.
  • Coordinate and secure transportation for Wyandotte County residents, upon discharge, to help them return to the community.
  • Provide education and communication with stakeholders regarding the AOT process and outcome tracking
  • Create promotional tools for AOT
  • Communicate regularly with the Care and Treatment Judge and DA’s office as needed regarding recommendations regarding AOT (Outpatient Treatment Orders) and other court matters
  • Create AOT care plans and participate in providing coordination for clients not yet receiving services (focus around any and all transitions mandatory)
  • Act as a liaison to all hospitals, court, probation, parole, primary care departments, hospital mental health environments (including ED), mental health treatment teams, SUD providers to support clients with an AOT or anyone post discharge
  • Works closely with ACT and Crisis Services for coordination of care

Position Requirements

Qualifications:

  • Bachelor’s Degree required; equivalent experience in community based mental health or mental health crisis work accepted in lieu of degree. Preference given to candidates with a Masters Degree in Social Work, Psychology, Counseling or Marriage and Family Therapy.
  • Clearance through the Kansas Child Abuse/Neglect Register, criminal background check, reference verification and all other screening and employment processes.
  • Familiar with treatment approaches and needs of individuals with severe and persistent mental illness.
  • Able to communicate effectively, orally and in writing.
  • Ability to establish effective working relationships with staff in various departments and community agencies.
  • Ability to provide direction for programming and planning purposes.
  • Ability to recognize and be sensitive to cultural and ethnic differences.
  • Ability to work autonomously in a multi-disciplinary setting
  • Must have dependable transportation.
  • Strong diplomatic skills to effectively articulate clinical impressions and advocate for client needs to a diverse group of professionals.
  • Crisis and Access Services requires strong team skills with a positive spirit, high energy and a “can-do” personality.
  • Strong care coordination skills, with a commitment to seeing things through.
  • Must have valid driver’s license and insurance.
  • Must be able to safely perform essential job functions, with or without reasonable accommodations.
