Position Midlevel Care Coordinator  
Description

Unique opportunity to provide evidence-based and outcome-focused care coordination for patients and internal treatment teams. As part of a member of an integrated multidisciplinary trauma-trained integrated care team, the Care Coordinator will coordinate care to improve or maintain mental, emotional, social, functional, and physical health of patients. The Care Coordinator will also offer professional consultation and education to team members and patients, as well as provide patients with advocacy, linkage, and care coordination with other health or social service agencies in the community as needed. Assist in development of integrated treatment planning and intervention development.

Qualified applicant will provide behavioral health screenings/assessments, and interventions with a trauma-informed approach, display excellent engagement skills with patients and team members, exhibit high energy, flexibility, and adaptability to variable paced environments, be well organized and respond to data-driven care gap and utilization information in coordination of patient needs.

 
Position Requirements

AA/BA required; CHW or LSW/LPC strongly preferred.

Valid Ohio Driver’s License, car insurance and willing to transport patients in own car required. Knowledge of community agencies and the skill to negotiate the system with the patient required.
