The Nurse Practitioner (“NP”) is responsible for the provision of primary health care services in compliance with the Standards of Practice for Registered Nurses in the Extended Class and the Expanded Nursing Services for Patients Act. This includes but is not limited to health promotion, counselling, disease prevention, diagnosis, treatment and referral to specialists and/or diagnostic testing. 

Program-Related Activities

  • Performs clinical tasks by conducting health assessments and diagnosis, therapeutics, health promotion, counselling, family health, community development and planning within the skills and knowledge of the Nurse Practitioner (NP) according to the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class (RN EC).
  • Practices autonomously, providing acute and chronic care management across the lifespan. Consults with CCHC physicians and/or other health care providers as needed.
  • Provides services at the Clinic with scheduled appointments and walk-in, at specified outreach locations, and through home/hospital visits as required.
  • Works in partnership with other members of the Primary Health Care Team to share responsibility for planning, provision and coordination of client care either in the home, the community or at the Health Centre.
  • Works within a health care framework which emphasizes health promotion and education, disease prevention and recognises the special risk factors (social, physical, psychological, etc.) which may affect the health of clients.
  • Acts in an ethical and professional manner in accordance with the code of conduct of the College of Nurses of Ontario and in accordance with the policies and standards (written and unwritten) of CCHC.
 
Position Requirements

Education

  • Undergraduate degree in nursing from a recognized university or equivalent.
  • Registration in the Extended Class with the College of Nurses of Ontario.

Professional Experience

  • Three to five years full-time NP experience in a community based setting or combination of community and hospital or public health settings; experience in a primary care setting, an asset.
  • Experience and interest in working with marginalized and diverse clientele, such as low-income, high-needs, newcomer, and a variety of multicultural communities.

Key Competencies

  • Thorough knowledge of and proficiency in current nurse practitioner physical assessment and treatment methods.
  • Ability to apply health promotion concepts, strategies, and philosophy.
  • Ability to develop and implement programs using an inter-disciplinary collaborative approach.
  • Demonstrated ability to model “client centred” service delivery.
  • High level of organizational and time management skills in a fast-paced environment. Demonstrated ability to prioritize, to be flexible, to meet deadlines.
  • Demonstrated proficiency in the use of computers and various software applications, including Microsoft Office programs, client database software, and electronic record keeping in a client services setting

Linguistic Profile

  • English is essential.
  • A third language (e.g. Arabic) relevant to our client population is an asset.

Salary Range $55.17/hr - $67.13/hr
