Schedule: Monday - Friday, 8:00 a.m. - 4:30 p.m., with occasional evening work as job duties demand.

Pay Range: $23.30 - $26.00 an hour based on education, experience, and credentials.

What You Will be Doing
We want people to join our team who share our values of love, excellence, respect, empowerment, and well-being. The ideal candidate will be primarily providing care in a clinical care/office setting. The LPN reports to a Registered Nurse and is responsible for working within the scope of their license to ensure that the physical and behavioral health needs of the patients are met and maintained on an individualized basis, following policies and procedures and current federal, state, and local standards, guidelines, and regulations.

Primary Job Responsibilities

  • Acting under the direction of a licensed physician or registered nurse, administering prescribed medications/treatments/immunizations, and drawing labs as needed.
  • Provide oversight and maintain medication accountability records.
  • Maintain medication equipment and supplies; perform skilled nursing procedures (e.g., applies and change dressings, catheterizes).
  • Assist in medical procedures.
  • Request, secure, and follow up on medical information of individuals.
  • Provide general nursing care to individuals.
  • Screen for communicable and nuisance diseases.
  • Observe and document conditions, responses to, and progress of nursing care treatment and medications, and report unusual developments to the supervisor or health care provider.
  • Provide education and training to individuals/caregivers/guardians/staff in health-related matters (e.g., nutrition, symptoms, hygiene, safety), as applicable to behavioral health/developmental needs.
  • Prepare and maintain appropriate documentation of service delivery and reports as required and ensure compliance with all reporting standards.
  • Participate as a member of the interdisciplinary team and work with other professional staff by contributing to planning, implementing, and evaluating nursing care plans to best support the needs of the individuals.
  • Provide consultation services to staff, individuals, parents, and significant others regarding health and safety-related issues, as directed by the RN or physician.
  • Gather health information for individuals served, including screenings, assessments, and vital signs.
  • Monitor individuals’ symptoms and effects of medications and share/report this information with appropriate team members, as applicable.
  • Assist the individual and staff as applicable in self-care techniques (to curb behaviors and limit medication use, etc.).
  • Manage behavioral crises and provide interventions as needed.
  • Identify and report any environmental or safety concerns to supervisor, team, and appropriate reporting body as applicable.
  • Participation in departmental meetings, committee meetings, and others as requested.
  • Perform other related duties as assigned.

Minimum Education/Qualifications

  • Must be at least 18 years of age.
  • Currently, in good standing LPN License with the Ohio State Board of Nursing.
  • Must be able to communicate in English, both verbally and in writing.

Required Licensure/Certification

  • First Aid, CPR - adult, child, and infant; or the ability to become certified in CPR/FA within 60 days of hire (training will be provided).
  • Covid-19 vaccine.

Preferred Education/Experience

  • One year of experience working as an LPN with individuals with behavioral health conditions or intellectual/developmental disabilities.
  • Preferred valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles; proof of auto insurance; insurable driving record (five points or fewer).

Special Skills and Requirements

  • Must be detail-oriented and have competent literacy and writing, documentation, communications, and interpersonal abilities.
  • Must be able to use tools and equipment as required – including pushing/pulling wheelchairs, lifting consumers with Hoyer lifts.
  • Must be competent in using computers, mainly working in an electronic health record, Microsoft Outlook, and Microsoft Office.
  • Successful completion of training in trauma-responsive care and crisis de-escalation/management within 60 days of hire.
