Case Manager BHH (per diem) 

Description

Looking for a per diem Case Manager for the Behavioral Health Home Department.

 

Job Summary:
Provide comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support services, and referral to community and social support services through person-centered care planning.

 

What You Will Do:

  • Engage consumers and their families
  • Facilitate and coordinate psycho-educational services (including groups) for consumers and their family members on a variety of topics relating to wellness
  • Offer on-site as well as outreach services (including transport of consumers)
  • Fulfill case management services
  • Ensure consumers have community support in place that fulfill a positive corroborative role
  • Communicate observations and information about the consumer in the consumer's service record and to team members
  • Engage in supportive counseling and crisis intervention
  • Participates in the assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery, and at discharge
  • Collaborate with team and collaborative treatment providers to establish comprehensive bio-psycho-social assessments and care plans
  • Participate in quality assurance activities and cross-functional action teams

 

What We Need to See:

  • Bachelor's degree in a related field
  • Two (2) years experience working within a health care or behavioral health setting; Experience working with adults with acute symptoms of mental illness and with chronic health care conditions.
  • Substance abuse certification certified i.e. CADC, LCADC
  • A valid driver's license and must be insurable by the agency's automobile insurance
