Behavioral Health Specialist

As a School Based Behavioral Health Specialist you will serve as liaison between CHC Providers and community mental health providers to ensure patients who have been identified through the school or CHC , in need of behavioral health services, receive services in a timely manner. You will work with schools to provide mental health support and resources to the school community (staff, students and families). This includes education and activities related to mental health. 

 This person would be working primarily in the East Moline School Districts. 

 

Job Specific Responsibilities:

  • Provide oversight for the development of a single treatment plan and lead the patient’s care team by coordinating care among all providers.
  • Coordinate with the care team to ensure appropriate health coaching and disease specific education are provided by appropriate staff, as needed.
  • Provide follow-up care to ensure that a successful connection is made, and that patients’ needs have been met.
  • Provide care coordination to patients in local hospital system to identify social determinants of health and improve the transition of care.
  • Monitor treatment adherence and response to medication and behavioral health treatment. If the patient is not participating in treatment, the care coordinator works with the patient/family/caregiver to identify and address barriers to treatment adherence.

 

Requirements:

Education: Bachelor's Degree in Social Work required; or Bachelor Degree in related social services area (defined as including Sociology or Psychology) with at least 30 semester hours in social work (twelve months of experience in the delivery of related social services in a social service or health field may be substituted in lieu of classroom training).

Experience: Minimum three (3) years casework/case management or related experience. Previous experience working with the mental ill and/or experience working with the long-term mentally ill population.
